What to Bring to Your First Visit:

To help us serve you better, we ask that you bring to your first visit the items listed below.

  1. Please be sure to bring your insurance card(s).
  2. Written order from your referring doctor.
  3. Authorization - many insurance companies require prior authorization to see a specialist. Patients are encouraged to contact their insurance provider to confirm whether an authorization is required for their specific carrier. If an authorization number is required by your insurance carrier, please fax it to (212) 746-8303, well before your appointment date. You may verify that it has been received by contacting the Call Center at (212) 746-1500.
  4. Complete the PDF icon General intake form or the PDF icon Women's Health intake form.
  5. Read and sign the Financial Policy.

For all Medical Release of Information, please complete the Authorization for Release of Medical Information  form and submit the completed form to

Weill Cornell Medical Records Department via email or Fax.

Failure to comply with this protocol will delay, or may even void your request. Please note, the retrieval of your medical records may take as long as 14 business days.

Make an Appointment:

Answers to Frequently Asked Questions

MyChart Messages:

  • If you send a MyChart message to your provider, it may take up to 2 business days to receive a reply.
  • If you have a new or worse condition that needs to be URGENTLY seen, please do NOT send a MyChart Message. Contact our office at 212-746-1500 to schedule an appointment.
  • If you need an immediate answer, call our office, and ask to speak with someone from our care team.
  • If it’s a medical emergency, please call 911.
  • No messages will be addressed after hours, on weekends or on holidays.

Results:

  • If you would like to speak with a physician to review your MRI or XRAY image results, please schedule a video visit or in person visit. If there is something urgent, you will be informed within 24 to 48 hours.

Orders and Refills:

  • If you require an order for Physical Therapy and it's been more than 6 months, you will need to make an appointment.
  • If you need a refill on a medication and it's been more than 3 months, you will need to make an appointment.

Forms:

  • If you require a form to be completed by our office, allow 10 business days for the completion of the form. Please upload the form via MYCHART portal or e-mail it to: [email protected]

Authorizations:

  • Procedural injections that require authorization, please allow up to 5 business days to receive an update from your insurance and our office.
  • If you need authorization for an external MRI institution, you will need to provide our office with the following information:
    1. Facility Name
    2. Full address where scheduled
    3. NPI number
    4. TAX ID number
    5. Telephone and Fax number

Specialty Pharmacy Medication:

  • Patients must consent and pay their copayment to their Specialty Pharmacy before the medication can be shipped to our office. Once the medication arrives, our office will contact the patient to schedule their injection appointment.

Miscellaneous:

  • If you would like us to fax out a prescription or order, please provide the office fax number where you would like it sent.
